Transition Program

From School to What Comes Next

The transition from high school into adulthood is exciting—but it can also be overwhelming for neurodivergent young adults and their families. At The Chase Academy Learning and Social Collective, our Transition Program helps young adults take the next step with greater confidence, independence, and support.

Rather than expecting students to suddenly navigate college, employment, relationships, responsibilities, and adult life on their own, we provide a supportive environment where they can continue developing the skills they need while actively working toward their individual goals.

There is no single path to adulthood. Our job is to help each student discover theirs.

College & Academic Support

Learning How to Be a College Student

For students pursuing college, technical school, certification programs, or other postsecondary education, getting accepted is only the beginning.

Transition students can receive ongoing support with the practical and executive functioning demands of college, including:

  • Understanding assignments and syllabi

  • Organizing coursework

  • Managing deadlines

  • Breaking larger assignments into manageable steps

  • Developing study strategies

  • Communicating with instructors

  • Navigating accommodations and support services

  • Managing schedules

  • Completing college coursework

  • Balancing school with other responsibilities

  • Developing greater academic independence

Our goal is not simply to help students complete an assignment. It is to help them develop the skills and confidence necessary to become increasingly independent learners.

Career & Employment Readiness

Discovering Strengths and Building a Future

Finding meaningful work begins with understanding yourself.

Transition students have opportunities to explore their strengths, interests, abilities, and career goals while developing practical skills for the workplace.

Support may include:

  • Career exploration

  • Résumé development

  • Job searches

  • Completing applications

  • Interview preparation

  • Workplace communication

  • Professional expectations

  • Time management

  • Organization

  • Workplace problem-solving

  • Self-advocacy

  • Building independence on the job

Whether a student's goal is competitive employment, supported employment, entrepreneurship, vocational training, or continued career exploration, we help them identify and work toward a pathway that makes sense for them.

Psychosocial Rehabilitation

Building Skills for Life Beyond the Classroom

Adulthood requires much more than academic or job skills.

Through Psychosocial Rehabilitation (PSR), eligible students can work on practical skills that support successful participation in everyday life.

PSR may address areas such as:

  • Communication

  • Social interaction

  • Emotional regulation

  • Problem-solving

  • Decision-making

  • Self-advocacy

  • Community participation

  • Healthy relationships

  • Personal responsibility

  • Coping strategies

  • Independent living

  • Building confidence in adult environments

These supports help students take what they have learned and apply it to the situations they encounter in the real world.

Building an Adult Life

Our Transition Program brings together the different pieces of adulthood rather than treating them as separate goals.

A student may be taking college classes while learning how to manage a schedule. Another may be preparing for employment while developing social and communication skills through PSR or may be working with Vocational Rehabilitation while exploring careers and becoming more independent in the community.

The pathway is individualized because the person is individual.

The Next Chapter

Graduation shouldn't mean that support suddenly disappears.

The Transition Program at The Chase Academy Learning and Social Collective provides a place where neurodivergent young adults can continue learning, growing, developing independence, and preparing for their futures.

We help students move from asking:

"What am I supposed to do next?"

to beginning to understand:

"I know where I'm going—and I know what support I need to get there."

Where Growth Meets Possibility.

Start Building the Next Chapter

Whether the next step is college, employment, vocational training, greater independence, or discovering what comes next, we're here to help young adults build the skills and confidence to move forward.
